This is how Shoto Todoroki’s scar was formed
To put ourselves in a situation, we must remember that Todoroki is the result of an arranged marriage, that is: Endeavor chose her partner Rei because of the Quirk she possessed; the ice one. This was done so that the children were born with unique abilities and after several attempts, it was like that. Shoto Todoroki would have the best of both parents, but not everything was smooth sailing. Endeavor wanted to put his son through a big workout, that’s when Rei objected. That’s where hell began.
As a result of Rei’s protection, Endeavor became abusive towards her., which caused him to be hated by Rei and as a consequence, also by Todoroki. All this abuse mentally broke Rei and in one of Todoroki’s appearances where she surprises her, his mother pours boiling water on his face; giving rise to the so characteristic scar that we know today.

Coincidentally, the water falls on the left side of Todoroki’s face, part that “belongs” to Endeavor if we take into account the hair color that the character has. This could be interpreted as all the damage caused to him and his family is Endeavor’s fault..
